Adults:

Any person 18+ must provide a valid/unexpired Photo Identification Document such as a state issued drivers license, state issued ID card, Military ID, or Passport.

State law requires you to present this information for every piercing appointment​

Minors:

The state of Nebraska requires a parent or legal guardian to sign consent forms and provide valid, unexpired documentation for the parent AND the minor. The person giving such consent must also be present for the procudure.

Required documentation for minors:
-Minor's birth certificate that also names the attending parent 
AND
(for all piercings other than earlobes)
-Minor's state issued photo ID, passport, or military dependent ID (last names must match parent and child)
Nebraska residents are able to obtain a state ID card at your local DMV.

-Legal name change documentation (if last names do not match)
-Court Approved Legal Guardianship documentation

AS OF JUNE 2023 THE STATE OF NEBRASKA REQUIRES A BIRTH CERTIFICATE FOR PIERCINGS ON ALL MINORS EVEN IF THEY HAVE A VALID PHOTO ID OR PASSPORT.
